New Patient Information

Please call our office at (209) 532-0511 to make an appointment. Every effort will be made to keep your appointment as scheduled. Office visits may need to be rescheduled if a doctor is involved in a medical emergency. You will be notified well in advance by our office, whenever possible, if appointment changes are necessary. If you need to reschedule your appointment time, as a courtesy notify the office as soon as possible, at least 24 hours in advance, to avoid 'failed appointment' charges and to make the appointment time available for others.

Information to Bring To an Office Visit: 

1 Please bring your insurance cards, list of medications you are currently taking with dosage, any medical records, or test results to your visit with us.

2. If your insurance requires a referral, please come prepared with your Primary Care Physician Referral and be prepared to pay your copayment as required by your insurance company.

3. If you are a new patient to our office, please arrive 30 minutes early to complete the paperwork.

How do I reach my physician after hours?
After the office closes our telephones are answered by a medical answering service. They will attempt to get you in contact with a physician when appropriate. For routine questions regarding billing, medications, etc., please call between 9:00 a.m. and 12:00 noon, and 2:00 p.m. and 4:00 p.m. All appointments with the doctors are by appointment only.
How do I get my prescription refilled? 
All prescriptions and authorizations for refills should be requested during our normal office hours. Please be certain you have all necessary drug refills before leaving the office after your visit. It is our policy to not refill your medications if rour pharmacy contacts us by phone unless it is an emergency. Phone refills are not usually necessary as we should provide you with enough refills of your medications to last until your next scheduled appointment. In addition, for reasons of safety, usually your prescriptions should be refilled by a single physician, your primary care physician.
How do I get copies of my medical records?
We need a signed consent form from you to release your medical records. The release must include your name, the date, and your doctor's name and the address where they are to receive the records.

The Consultation
The initial consultation requires about ninety minutes and includes a electrocardiogram. The doctors will examine you and then go over the results of the history and physical examination. Subsequent appointments may be made to order further tests or Xrays, and often a return visit will be scheduled. Return visits require about 30 minutes.

How to Prepare For Your Examination or Visit
Please bring with you at the time of your initial appointment all medical records in your possession and your medicines in their original containers showing the name and strength of each drug. Include a list of your medical complaints as well as previous medical illnesses or surgeries and their dates.

Financial Policy
Our staff welcomes you to our practice and thanks for allowing us to provide your medical care. We are committed to improving your health with the latest in technology, using a scientific approach to medical care which includes compassion. We ask your commitment to be responsible for payment of these services in a timely fashion. Full payment of your estimated portion is due at the time of service by cash or check.

For insurance plans for which we have a provider agreement, co-pays and deductibles are due at the time of service. For insurance plans for which we are NOT a participating provider, your balance is your responsibility. We will bill your insurance. If we accept assignment, and if the insurance company has not paid in 60 days, the balance will be due immediately.

Be aware that some services may not be covered by your insurance. Medicare Patients: we are a participating provider, which means we agree to accept the charge and determination of the Medicare carrier as the full charge. The patient is responsible for the deductible, co-insurance, and non-covered services.

Usual and Customary Rates: You are responsible for payment regardless of the insurance company's determination of usual and customary rates.
